The role!

Position: Residential RN Healthcare Coordinator
Location: Wakefield, MA
Pay rate: $68,640 annual salary 
Hours: Full-time, 30 hours per week                                                                                                                    Schedule: This position will typically operate on a Monday – Friday schedule; may be required to work evenings and weekends in emergency healthcare needs; must have flexibility as schedule may vary to meet needs of agency and the Individuals we support

Summary & Responsibilities

The Health Care Coordinator will be responsible for the oversight of the overall healthcare and well-being of individuals who reside in our Residential Program group homes and independent apartment settings. We are looking for someone with a strong background and experience in working with adults with Developmental and Intellectual Disabilities.
  • Staff training on specific treatment protocols
  • Consultation for individual medical needs
  • Perform MAP trainings and recertifications
  • Perform MAP and medical file audits
  • Residential home and hospitalization visits as needed
  • Attend administrative meetings and complete related paperwork
Qualifications
  • Must have a current Massachusetts license as a Registered Nurse (RN).
  • Must have at least 3 to 5 years’ experience in a community health setting; preferably with adults with developmental and intellectual disabilities.
  • Certified MAP Trainer is a plus but training is also available
  • Successful candidates will be required to complete a background check including references, CORI, and fingerprinting. 
  • A passion and dedication to supporting our people is a must
  • Valid Driver’s license, reliable transportation, and acceptable driving record
  • Must pass company background and reference checks
